Patients over 75 years

Patients aged 75 years and over who have not had a consultation in 12 months will be requested to attend an appointment to review their health and wellbeing with a GP.

If you are not able to attend the practice a home visit can be arranged.

If you are aged 16 years and over and a GP has not seen you in the last 3 years, the practice will reach out to you for a review of your health and wellbeing.
